CTP Incubator

Impact:    Expand opportunities for post-transition age young adults with IDD

Year 1 Objectives:

  • Landscape analysis
  • Establish cohort of programs to support CTP development activities 
  • Liaise with DRS to maximize funding for CTP students
  • No funding to individual colleges in Year 1, but potential funding for individual colleges in years 2 and 3 when Coleman’s budget more than doubles.

  • CTP FAQs Document
  • Website content
  • CTP Statewide Directory
  • Year 1 Project Impact Report
  • Slide deck template on CTP presentation available for coalition members to use with affiliated audiences

Dignity In Pay Alignment

Impact:    Shape emerging state policy

Year 1 Objectives:

  • Monthly coordination meetings with ICDD team to determine alignment opportunities 
  • On-going feedback sessions with individuals, providers, and families to inform DIP efforts
  • Support to ICDD in identified areas to further reach of communication, feedback, and implementation

Year 1 Deliverables:

  • Impact Summary Report
  • DIP Infographic for dissemination to families, individuals with IDD and provider community
  • Informational webinar recorded and available for on-demand viewing

Behavioral Health Clinic Awareness + Expansion

Impact:    Address widely acknowledged unmet needs for IDD population

Year 1 Objectives:

  • Develop framework for provider education and awareness regarding BHC opportunity
  • Promote communication and information exchange among current BHC providers
  • Expand family and consumer awareness regarding access to BHC resources
  • Facilitate expansion of BHC network 
  • Identify challenges and opportunities within BHC model that may require regulatory, administrative and/or legislative intervention

Year 1 Deliverables:

  • BHC Resource Manual for provider agencies
  • BHC Infographic 
  • Informational webinar recorded and available for on-demand viewing
